Diagnostic Assessments (DA)

For those needing a diagnostic assessment to begin services, maintain insurance coverage, or for whatever reason it is needed.

Do you or someone you know need a Diagnostic Assessment (DA)?

The diagnostic assessment (DA) is necessary to determine a member’s eligibility for mental health services through insurance.

The process entails about an hour in length session to gather information and assess someone's mental health.

If found, a mental health diagnosis is given at the end or following session.

Do note, if after the assessment is complete and both parties agree that working together in further therapy is appropriate then future sessions can be scheduled at that time.

As a licensed independent clinical social worker (LICSW), I am trained to diagnosis all ages!

I can conduct the following types of DAs:

DC: 0-5 (Birth to 5 years old)
3 sessions total

Children's DA (6 to 18 years old)
1 or 3+ sessions

 Adult DA (18+)
1 or 3+ sessions

Some exclusions apply:

To formally diagnosis Autism (ASD), Attention Deficit / Hyperactivity Disorder (ADHD), or Learning Disabilities, it is best to have a psychological assessment completed.

This type of an assessment can be completed by a psychologist or psychiatrist.

It is recommended you reach out to your insurance provider and ask for a recommended approved provider.

Helpful Information regarding Payment

  • Sessions are 1 hour in length 
  • Rate is $200 per session 
  • (Insurance Accepted: UBH, Medica, South County Health Alliance, UCare & Medical Assistance)

Frequency / Need of DAs

* Children need an updated DA every year

* Adults need an updated DA every 3 years

* Anyone who is looking to start mental health services must have a DA completed before starting services
(if you are looking to start therapy services, typically your therapist will conduct a DA prior to starting services)
